 A past experience I had with a survey, was in a preflight interview for my pilot training. The questions were about my life. So medical evaluations, exercise, medicine, grades, everything. The survey audience was the FAA and my CFI or certified flight instructor. A survey for addiction in the pandemic would be far different and less invasive than the one I took as a pilot trainee. That being said I believe a survey would serve to be ineffective as it would be hard to gather usable information and valid data.
